‘22. Tooth knocked out’

Case 22: Tooth knocked out

A three year old has been brought in. She fell onto her face at home and has knocked out an upper incisor. Her mother has placed this in a cup of milk and brought it with her. She asks you about reimplantation.

1.  What would you advise?

The general recommendation is not to re-implant milk teeth.

2. Give a basis for your answer

Primary teeth may ankylose to the underlying bone if reimplanted. This may lead to problems with the primary tooth separating later when the underlying permanent tooth needs to erupt. Furthermore there is no long-term cosmetic harm if primary incisors are lost – the permanent teeth will grow normally. Hence reimplantation is not recommended.

3. What should be the next action with the avulsed tooth?

Place it under the child’s pillow for the tooth-fairy to collect. (should be worth £2 at least!)
